Hospice care through Principle Choice Home Healthcare and Hospice is coordinated across a full care team. Physicians, nurses, therapists, social workers, chaplains, aides, and volunteers communicate regularly to ensure care remains consistent and responsive.
We also help coordinate medications, medical supplies, and necessary equipment related to hospice care as prescribed, reducing stress for families and caregivers.
We believe that the family and caregivers need support with information, reassurance, and guidance throughout the hospice experience. This support is designed to help loved ones feel informed and confident in the care being provided. We remain present as a consistent point of connection, helping families feel less alone during a time that can often feel uncertain.
While our entire hospice care team coordinates care for the patient, we are also there as support to the family navigating a hard transition. Our Medical Social Worker also provided emotional support and short‑term counseling for patients and families. They assist with care planning and help connect families with community resources. Social services may also include guidance related to living arrangements, advance planning, and practical or financial concerns, helping families feel informed and supported.

We help guide patients and families through the hospice admission process with clarity and sensitivity, ensuring they feel informed and supported from the beginning.
